   Please contact your insurance prior to your visit to verify we are in network -- many times you can call member services on your card or search for our physicians on their insurance website. If you do not verify services, we may be able to see you as a self-pay client instead.

 There may be an additional fee up to $25 for completion of forms for use outside of the patient's medical record including:

• Camp Forms
• Family Medical Leave Paperwork
• Immunization Records
• Sports Physicals
